Some inconsistent hyphenation (e.g., toenail/toe-nail) was left as in the original.

Chapter XIV in the Table of Contents was wrongly shown as page 123, and this was changed to 133. Likewise chapter XLV in the Table of Contents was wrongly shown as page 425, and this was changed to 426.
